The Field Nurse (LPN/LVN) conducts in-home/field-based visits to address urgent clinical issues with Landmark patients. While on-site, the Field Nurse will evaluate patients and communicate, via telephone or secure video, with the supervising Landmark provider. The Landmark provider will direct the evaluation and care provided. All activities will fall within the employee’s scope of practice as set by the State they are practicing in.   *Assigned Shift - Saturday and Sunday 5pm - 2am

Landmark recognizes that Behavioral Health dramatically can impact Medical Health and this position collaborates with the Chief Behavioral Health Officer, Associate Director of Behavioral Health, Regional Medical Director, PCP/Psychiatrist, interdisciplinary team and other health professionals, to develop comprehensive member goals of care to fully integrate medical and behavioral health care for optimal outcomes and quality of life.
