At Landmark, our interdisciplinary teams collaboratively manage our complex patient panels. These teams are led by Physicians, Nurse Practitioners and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health and other employed team members.
As a Provider, you will manage a small panel of high-complexity patients in partnership with an RN Nurse Care Manager. We value quality over quantity; at Landmark you will be able to invest more time in your patient visits than you are normally able to spare in a traditional fee-for-service model. Our home visits typically last 30 minutes to an hour, which grants you time to complete medication management, disease education, and collaborate with your extended Landmark team or with community clinicians.

At Landmark, our interdisciplinary teams (IDT) collaboratively manage our complex patient panels. These teams are comprised of individuals with diverse backgrounds, and includes Physicians, Nurse Practitioners, and Physician Assistants RN Nurse Care Managers (NCMs), Social Workers, Ambassadors (Care navigators), Pharmacists, Dietitians, Care, Behavioral Health and others.
The Clinical Supervisor is responsible, as part of the Market Leadership and care teams, for the overall patient care management process. The position reports to the Health Services Director and provides oversight, guidance and support for the processes related to the development, implementation and outcomes of member care plan. The position provides support for the day to day operations as well as for external meetings with facilities, providers, and health plan partners. The Clinical Supervisor will manage direct reports and will be responsible for people and performance management to maximize the effectiveness of the interdisciplinary team. The Clinical Supervisor will also help to manage inbound clinical calls to ensure appropriate and timely response to Landmark patients.

The Urgentivist primarily provides medical care and management to members served by the Landmark program who request urgent visits to the NCM on call from 8:00 AM to 5:00 PM Monday – Friday (Non-Holiday). This position prescribes appropriate diagnostics and interventions to avoid unnecessary transfers and acute admissions. Communicates and collaborates with the assigned Landmark primary provider and interdisciplinary team in achieving patient’s care goals.

The Community Health Advocate will be responsible for enrolling patients into the Landmark Health program in a face-to-face environment. This role is required to engage patients that have either requested to be contacted in a face-to-face setting, or have proven to be unreachable through telephonic and/or mail communications.
Responsibilities include developing local strategy and tactics to reach the member population, including working with the General Manager and Regional Medical Director on PCP and provider outreach, and developing outreach strategies to community based services. This role is responsible for regular reporting on production and progress.
