As a Landmark Physician, you will act in a dual capacity as a practicing Physician providing care to patients (60%) and as a “Pod Leader” providing oversight and guidance for clinical team members (40%). This allows you to gain valuable leadership experience while continuing to personally contribute to the health and wellness of patients.


You will visit your panel of patients in the comfort of their own homes and spend all the time needed to ensure your patients’ health issues are addressed. You create a friendly and trusting relationship with each patient and his or her family. By observing your patients in their homes, you will gain valuable insights into aspects of their lives – sometimes subtle, but always important – that otherwise may never be revealed in a traditional care setting.


In this Physician (Pod Leader) role, you will direct the efforts of a collaborative, multidisciplinary clinical team who works in the field with you. You will have the resources of our custom electronic medical records (EMR) program, comprehensive mobile medical technology system and dedicated technical support department.

As a Physician (M.D. or D.O.), your duties will be to:

  • Function as a day-to-day clinical leader, providing decision support to Advanced Practice Providers (APPs) and directing the multidisciplinary team
  • Perform 4-6 preventive visits daily to optimize chronic conditions, assess home environment, educate patients and caregivers, and develop proactive care plans
  • Perform urgent care visits in the home to avoid unnecessary ED transfers and hospital admissions
  • Leverage the support of Nurse Care Manager, Behavioral Health specialists, Social Workers, Pharmacists, and Dieticians to meet patients’ medical, biopsychosocial, and financial needs
  • In situations where there is no existing PCP for the patient, assume responsibility as PCP to drive care and continuity for patients
  • In situations where there is an existing PCP for the patient, help to co-manage the patient with the PCP and serve as an extension of clinical care into the home
  • Coordinate with other Physicians across the continuum of care, including PCP, hospitalist, and SNF providers to smooth transitions and prevent readmissions
  • Coordinate and offer medical direction to community-based organizations touching the lives of our patients, including housing and caregiver agencies, health plan contracted social work services, home health, adult day health centers, and behavioral health







  • Must be board certified at the time of hire or able to obtain board certification during the eligible timeframe (5 years following completion of residency or other training) – preference for Internal Medicine, Family Medicine, or Emergency Medicine
  • Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited educational institution
  • DEA registration
  • Experience managing complex, polychronic patients and comfort with high acuity – working with geriatric patients is a plus
  • Dedication to transforming care for the sickest and frailest in a marginalized population
  • Eagerness to work with a diverse and multidisciplinary team
  • Access to reliable transportation to conduct patient visits required; if you are driving a vehicle, you must comply with all the terms of the Landmark Motor Vehicle Safety policy


In order to work with potentially vulnerable patient populations and to keep employees healthy while caring for patients, all patient-facing employees of Landmark must provide evidence of the following required immunizations/vaccinations as a condition of employment: TB, MMR, Tdap, and Influenza.
