    The Community Outreach and Patient Engagement Manager is responsible for designing, setting and executing local outreach strategy, and for ensuring that their market meets or exceeds annual engagement targets. In addition, the Manager will have both HR and performance responsibility for the local market Community Health Advocate (outreach team) employees. Lead regular meetings with the CHA team, and local market leadership, prioritize local outreach activities and allocate local outreach resources. This supervisory role will require close performance management of individual local Community Health Advocate employees. In addition, the Community Outreach and Patient Engagement Manager will liaise closely with the Market Leadership Team to set, define or modify local outreach strategy, monitor on a weekly basis, local outreach productivity and performance relative to budget, and coordinate on the resources required for specific executive meetings with community partners

     

    This position is responsible for bringing insight from the market back to the local market leadership team and the Landmark marketing team to improve our communications with patients, physicians, and other community partners to enhance our relationships and improve our patient engagement. Manages community partnership CRM that will track interactions and relationships with community partners. In addition the Manager will be responsible for development of quarterly and annual reports to major community partners that outlines the value Landmark has provided to mutual patients.

    Responsibilities

    • Source, establish and maintain collaborative relationships with strategic partners including medical groups, health systems and hospitals, skilled nursing facilities, retirement communities, assisted living communities, and other community organizations
    • Develop and implement patient retention strategies working with local leadership and marketing teams
    • Develop and execute market wide data driven strategy to enroll eligible patients in the Landmark program to exceed market targets in conjunction with the market leader
    • Manage local community outreach team including training, coaching, and performance management
    • Partner effectively with to serve as an expert resource to the team, while working closely with local market leadership to drive the effective execution of monthly engagement strategies and tactics 
    • Serve as a member of the market leadership team responsible for providing concise and effective updates and during weekly management team meetings
    • May work flexible or extended hours to accomplish business goals
    • The position offers opportunity for above-market compensation, including substantial monthly bonus opportunity tied to individual and market production and performance
