    Our model is finding success throughout the country; we are now the nation’s largest risk-based, in-home medical group, with operations in six markets and four states across the country.

    At Landmark, our interdisciplinary teams collaboratively manage our complex patient panels. These teams are led by Physicians, Nurse Practitioners, and Physician Assistants, with supporting care provided by RN Nurse Care Managers, Social Workers, Pharmacists, Behavioral Health and other employed team members.


    Community Health Advocate will be responsible for scheduling patients into the Landmark Health program through both telephonic and face-to-face engagement. The focus of this role is to schedule patients for their first visit with a Landmark provider.  Responsibilities include working with the Supervisor, Outreach and Engagement to develop local outreach strategy and tactics to reach member population, community providers and facilities.  This role is responsible for regular reporting on scheduling productivity and engagement outcomes.


    Overview of Job Duties

    Engagement/Relationship Building

    • Enroll patients into the Landmark Health program, primarily through face-to-face engagement strategies
    • Serve as a relationship development liaison between Landmark and hospital personnel, patients, physicians, community organizations, retirement communities, assisted living, primary care, and skilled nursing facilities.
    • Maintain ongoing communication with hospital and SNF case managers, discharge planners, physician office staff, and other community referral sources; conducting frequent presentations, both formally and informally, to build a strong network of health plan
    • Provide/coordinate current and potential referral sources with educational in-services.
    • Establish & maintain relationships with current and potential referral sources and assure that our service and delivery of care meets and exceeds their expectations
    • Attend hospital case manager/discharge planner meetings/case conferences to discuss Landmark patients.
    • Communicate information regarding the patient to the clinical supervisor and/or longitudinal provider; making Landmark Health administrative staff aware of additional patient needs


    • Build and grown exceptional relationships and influence key decision makers with referral sources in Hospital, SNF, PCP, Healthplan staff and other Community Agencies.
    • Meet and/or exceed engagement goals as they relate to quarterly growth goals; identify additional opportunities for further engagement.
    • Identify innovative engagement strategies to enroll patients into the Landmark Health program
    • Make weekly visits; follow-up calls and presentations to current and potential referral sources
    • Consistently meet established outreach and call objectives, while completing documentation into company CRM, and EMR.
    • Proficiency with Excel; ability to review reports, gather and analyze data regarding territory and referral patterns.
    • Proven success with time management and territory organizational skills


    • Maintain and ensure the confidentiality of all Protected Health Information
    • Flexibility to work extended hours to accomplish business goals
    • Promote service excellence and display the highest level of business professionalism always.
    • Ability to work within an inter-disciplinary team environment


    • 2+ years of related experience, with prior involvement in a community advocate or Medicare Advantage sales role preferred
    • Bachelor’s Degree in public health, health communication, or related field preferred
    • Demonstrated sales, networking and negotiation skills
    • Demonstrated public speaking and presentation skills
    • Excellent interpersonal skills and proven ability to adapt to varying communication styles
    • Demonstrated ability to work in an entrepreneurial environment with minimal supervision
    • Proven experience communicating with seniors
    • Ability to multi-task and work in a dynamic and environment
    • Conversational understanding of medical terminology
