    Join us in transforming healthcare!

Landmark Health was created to transform how healthcare is delivered to the most medically vulnerable members in our community.  Landmark Health is a medical group that provides home-based medical care to chronically ill patients, many of whom are ill-equipped to navigate our overwhelming healthcare system.

Landmark extends the mission by transforming care delivery for patients that reside in skilled nursing facilities (SNFs) and long-term care facilities. Landmark embeds high-quality primary care providers (MD and/or APPs) in facilities as dedicated support to treat residents in-place. By placing primary care resources in facilities, we aim to deliver better health outcomes for patients, improve facility performance on quality metrics, and improve patient and family satisfaction.

We are looking for providers who are passionate about improving the quality of care for patients residing within facilities and helping facilities improve and maintain high performance on quality metrics and standards.

As a Landmark provider you will manage a panel of patients within a designated facility. You will work closely with the nursing staff at the facility, the Medical Director, and with other members of the patient’s care team in managing patient care. Care will include performing admission H&Ps, conducting regular interval and medically necessary visits, managing the patient’s plan of care, follow-up with patient’s family, and day-to-day communications and follow-up with facility staff. You may also be involved in the development and/or implementation of medical management policy, clinical protocols, and quality management programs. As a provider, you will be expected to perform the following activities:

    • Establish and maintain effective relationships with patients, their family and legal representatives, facility staff, and other collaborating physicians/providers.
    • Diagnose, prescribe, and treat within the scope of the Nurse Practitioner and consistent with standard of care.
    • Collaborate with facility staff to enable medical necessity determinations for requested medical services.
    • Following the visit, promptly document and communicate any pertinent information to the facility staff
    • Understand and comply with long-term care policies and procedures, including those that regulate nursing facilities and assisted living communities.
    • Participate in telephonic on call rotation

As a Provider at Landmark, you will benefit from the following:

    • Excellent compensation and benefits including a robust bonus program, market-leading Paid Time Off (PTO), CME reimbursement, mileage reimbursement, child care benefits, and more.
    • A team-based clinical environment, where you will find mission-driven clinicians who are passionate about fixing our broken system of care.
    • An outcomes-oriented model, where more value is placed on high-quality clinical outcomes than on productivity.
    • Geriatric expertise – Landmark invests significantly in provider education, with a particular focus on geriatrics.
    • Full Time

        • Completion of Nurse Practitioner or Physician Assistant accredited program
        • Minimum of two (2) years clinical experience, desired
        • Current unrestricted state license and current state driver’s license
        • Knowledge of CPT, ICD-9 and HCPC codes
        • Knowledge of clinical standards of care
        • Awareness of UM standards, NCQA requirements, CMS guidelines, Milliman guidelines, and Medicaid/Medicare contracts and benefit systems is helpful
        • Passion for geriatric patient care
