    Landmark Health is the national leader in home-based care for complex patients. Over the next several years, Landmark is poised for tremendous growth. To do this successfully, we need to build a best-in-class Implementation team that can support the structured scalability of the Landmark model. The Director, Operations & Implementation will be responsible for driving all new market implementations in the Eastern Region for Landmark. This position reports to the VP, Operations & Implementation.


    • Oversees new health plan implementations, working with Director of Client Operations and Project Manager, to insure key deliverables, milestones, and timelines are met with health plan implementation

    • Build strong relationship with health plan peers and act as the day-to-day operational lead for the relationship

    • Assists operational and medical leadership in recruiting training and on-boarding of initial hires within new local markets, including the market leadership team as well as clinical team members

    • Serve as the Operational leader in PCP and community outreach for each new market, partnering closely with the clinical leadership to identify key stakeholders and build positive relationships within the healthcare ecosystem

    • Serves as an Interim General Manager for the new market until the full-time General Manager is hired and trained, leading the market team and market strategy

    • Accountability for meeting early market metrics including:

      • Recruiting and hiring targets

      • Patient engagement targets

    • Implementation of clinical best practices

    • Own and drive continual improvement of the new market implementation process and playbook, working towards the goal of enhanced scalability for the company

    • Serve as a liaison between national operations and the regional markets, establishing best practices and creating the infrastructure and processes to facilitate their dissemination and standardization across markets

    • Contribute to high-level strategic planning as a member of the Senior Management Team

    • Help build a culture of continuous operational improvement across the company

    • Travel required – expected to be ~50% travel, with high travel required for 2-3 month stretch during each new market implementation


    • Bachelor’s Degree in healthcare related field or business administration, MBA/ advanced degree preferred 

    • 5+ years of business management and / or relevant consulting or professional services experience 

    • Proven track record of driving operational projects from conception to implementation to refinement 

    • Excellence in organizational and project management with the ability to lead multiple complex projects 

    • Track record of leading without authority, demonstrated ability to influence others and drive cross-functional results 

    • Analytic and decisive decision maker with the ability to prioritize and communicate objectives and tactics necessary to achieve organizational goals 

    • Action-oriented, entrepreneurial, flexible, and innovative approach to operational management 

    • Solution-oriented mindset and work approach 

    • Charisma and a natural ability to connect with others 

    • Excellent communication and interpersonal skills, with the ability to be personable yet persistent
