  • Overview

    Ensures the safe and effective use of medications for Landmark patients by providing medication therapy management including medication reconciliation, assessing for optimal drug product selection, dosage regimen design, effective administration and drug therapy monitoring services. Reviews inpatient and outpatient drug therapies when appropriate, and works with Landmark clinicians and multi-disciplinary team to find alternatives when needed, to ensure the best care possible is being given and to maximize clinical and financial outcomes.

    Responsibilities

    • Prevent, detect, and resolve medication-related problems and to make appropriate evidence-based, patient-centered medication therapy recommendations as part of the multi-disciplinary team.
    • Determine and resolve any medication therapy problems including drugs without indications, medical conditions for which no therapy is prescribed, therapy inappropriate for medical condition, incomplete immunization record, inappropriate dose, form, duration, schedule, route of administration, method of administration, therapeutic duplication, medication allergies, and clinically significant drug-drug, drug-disease, drug-nutrient, or drug-laboratory test interactions.
    • Prioritize level of care based on critical needs and performs home visits for patients as needed.
    • Provide concise, applicable, comprehensive, and timely responses to requests for drug information from patients and health care providers.
    • Educate patients and families on medication indications.
    • Recommend generic substitution when available and appropriate.
    • Participates with multi-disciplinary team to identify high-cost and high-use agents within defined area of practice and participates in financial management initiatives (generic substitution, cost-containment, etc.).
    • Actively participates in the development and implementation of performance/quality improvement and patient safety activities with the Landmark Team.
    • Identify potential technical or patient care issues and work with Landmark staff to make recommendations to avoid issues.
    • Communicates effectively with multi-disciplinary team and outside providers.
    • Represent Pharmacy/service areas on institutional/ practice area committees and quality support teams.
    • Provide in-services/lectures to Landmark multi-disciplinary team.
    • Participates in mentorship and professional development of newly hired Landmark pharmacists.
    • Complete required continuing education credits and submit pharmacist license renewal to State Board of Pharmacy on time.
    • Enhance professional and technical knowledge/competency by attending class work/other training, seminars, and/or journal clubs.
    • Some travel required

    Qualifications

    • Bachelors in Pharmacy or Pharm.D. Degree.
    • License/Certification: Licensed to practice within state or eligible within 60 days of hire
    • Minimum of two (2) years clinical experience, desired.
    • Driver’s license without restrictions and with good driving record.

    WORK ENVIRONMENT

    Below is a description of the work environment that the employee will encounter while performing the essential functions of the job. 

    1. Will require regular travel to patient homes for visits as needed
    2. Noise level is usually moderate
    3. Expected to work normal business hours between 8am-5pm
