Responsibilities

Position Overview:

In this high-profile role, the Director of Health Services Operations and Population Health is an integral role of the Landmark Massachusetts Leadership team, and manages,. This role manages, trains and develops in-home interdisciplinary team members and works closely with the Regional Medical Director, Operations Manager, and Clinical Supervisors to drive quality and population health management initiatives including: closing HEDIS quality gaps, optimizing patient panels, and coordinating resources across our interdisciplinary team. The ideal candidate will have comfort working cross-functionally, developing data driven strategies to improve the quality of patient care, and collaborating with our health plan partners and other community partners.

 

What you'll be doing:

 

  • Manage, train, and develop interdisciplinary team members including social workers, healthcare ambassadors, and nurse care managers
  • Work closely with the Regional Medical Director, Operations Manager, and Clinical Supervisors to drive quality and population health management initiatives including: closing HEDIS quality gaps
  • Develop and manage market quality program (i.e., closure of HEDIS gaps)
  • Manage relationships with vendors, health plan case management, and community organizations.
  • Optimizing patient panels, and coordinating resources across our interdisciplinary team. 
  • Work cross-functionally, developing data driven strategies to improve the quality of patient care, and collaborating with our health plan partners and other community partners.
  • Spot and implement opportunities to improve clinical operations of the market to improve quality and population health outcomes
  • Establish processes for inpatient, Skilled Nursing Facilities / Long Term Care (SNF/LTC) and post hospital discharge programs within the standardized across market processes.
  • Manage key regulatory and compliance processes including CLIA, complaints and grievances, and HIPPA.
  • Attend and actively participate in contracted health plan IDT, Clinical Operations, JOC’s and other operational meetings.

Qualifications

What we're looking for:

  • RN License or Bachelor of Science in Nursing is highly preferred, MBA is a plus
  • 5+ years track record of managerial and leadership success in a health care setting
  • Managerial experience outside of nursing/case management is a plus
  • Quality programs, Risk and/or Population Health experience highly desired
  • Exposure to clinical quality measures (HEDIS, Star) and willingness to learn more
  • Experience with healthcare metrics and analytics is a plus
  • Health Plan and/or Managed Care experience is a plus
  • Demonstrated critical thinking and problem-solving experience
  • Excellent organizational and project management skills
  • Experience with MS Excel and PowerPoint is helpful
