Landmark Connect is a division of Landmark Health. Landmark Health was created to transform how healthcare is delivered to the most medically vulnerable members in our community.  Our medical group provides home-based medical care to chronically ill patients, many of whom are ill-equipped to navigate our overwhelming healthcare system.  

 

Landmark Connect extends the mission of Landmark Health by transforming care delivery for patients that reside in post-acute facilities (e.g., Skilled Nursing Facilities). Landmark Connect embeds high-quality primary care resources (MD and/or APPs) in facilities to provide dedicated support and in-place care of residents. By placing primary care resources in facilities, we aim to deliver better health outcomes for patients, improve facility performance on quality metrics, and improve patient and family satisfaction.

 

The Market Supervisor is the day-to-day leader in their market owning the market operations and driving completion of administrative activities. The Market Supervisor reports to the Executive Director for the region and will work closely with them to manage day-to-day operations, identify opportunities to improve efficiency and quality, support providers in their market, and develop close relationships with key local partners. The Market Supervisor will work closely with Landmark Connect providers in the market to ensure that all providers are able to operate efficiently and provide excellent patient care to deliver on our mission.

The Market Supervisor is a pivotal member of the Landmark Connect team. As a Market Supervisor, you will use your critical thinking, written and verbal communication, customer service, and administrative skills to ensure that our providers can focus on delivering the highest level of care to patients and our health care partners.

Responsibilities

Operational Oversight: Responsibility for the operational functioning of the market, including but not limited to:

  • Serve as the day-to-day support for provider administrative-type questions, concerns, and challenges
  • Oversee provider schedules in EMR
  • Provider call schedule oversight and coordination
  • Administration of PTO and payroll system
  • Support standing facility meetings (e.g., weekly provider-facility team huddles, monthly Joint Operating Committee meetings)
  • Oversee day-to-day billing questions and patient insurance verification process
  • Set up and manage provider patient schedules
  • Manage all aspects of operations associated with on-boarding new hires

Operational Implementation and Improvement:

  • Develop a comprehensive understanding of daily operations, identify operational roadblocks and process gaps and implement all operational improvement as necessary.
  • Problem solve issues the clinical team is facing.

Metric Tracking and Reporting:

  • Oversee operational performance reporting for the market and ensure timely communication of performance gaps and opportunities for improvement
  • Own and drive reporting on operational performance metrics for the market including regular review of all relevant KPIs, qualitative and quantitative analysis, and synthesis and presentation of results.
  • Translate performance metrics into actionable operational recommendations for improvement – partnering and working collaboratively with others across Landmark.

 

The position requires a mix of on-site presence at the facilities and presence in an office. Provide flexibility to work from home on an as needed basis.

 

Qualifications

  • Bachelor’s Degree in healthcare related field or business administration preferred
  • Healthcare experience preferred, not required for the right candidate who can learn quickly
  • Excellence in organizational and project management with the ability to lead multiple complex projects
  • Action-oriented, entrepreneurial, flexible, and innovative approach to operational management
  • Solution-oriented mindset and work approach
  • Ability to work in an autonomous environment
  • Charisma and a natural ability to connect with others
  • Excellent communication and interpersonal skills, with the ability to be personable yet persistent
  • Proficiency in Excel preferred
