Our mission at Landmark Health is to transform care delivery for patients that reside in skilled nursing facilities (SNFs) and long-term care facilities. Landmark Health embeds high-quality primary care providers (MD and/or APPs) in facilities as dedicated support to treat residents in-place. By placing primary care resources in facilities, we aim to deliver better health outcomes for patients, improve facility performance on quality metrics, and improve patient and family satisfaction.
We are looking for providers who are passionate about improving the quality of care for patients residing within facilities and helping facilities improve and maintain high performance on quality metrics and standards.
As a Landmark Health provider, you will manage a panel of patients within a designated facility. You will work closely with the nursing staff at the facility, the Medical Director, and with other members of the patient’s care team in managing patient care. Care will include performing admission H&Ps, conducting regular interval and medically necessary visits, managing the patient’s plan of care, follow-up with patient’s family, and day-to-day communications and follow-up with facility staff. You may also be involved in the development and/or implementation of medical management policy, clinical protocols, and quality management programs.
