Because many of our patients are frail and elderly, we deliver care primarily in their home Our clinical teams spend quality time caring for a smaller number of high-risk patients, granting patients the respect, compassion and care they deserve.

At Landmark, our interdisciplinary teams (IDT) collaboratively manage our complex patient panels. These teams are comprised of individuals with diverse backgrounds, and includes Physicians, Nurse Practitioners, and Physician Assistants RN Nurse Care Managers (NCMs), Social Workers, Ambassadors (Care navigators), Pharmacists, Dietitians, Care, Behavioral Health and others.

The Clinical Supervisor is responsible, as part of the Market Leadership and care teams, for the overall patient care management process. The position reports to the Health Services Director and provides oversight, guidance and support for the processes related to the development, implementation and outcomes of member care plan.  The position provides support for the day to day operations as well as for external meetings with facilities, providers, and health plan partners.  The Clinical Supervisor will manage direct reports and will be responsible for people and performance management to maximize the effectiveness of the interdisciplinary team. The Clinical Supervisor will also help to manage inbound clinical calls to ensure appropriate and timely response to Landmark patients.


  • Quality oversight and operational support: Provide quality oversight and operational support for the Health Services Director and Market Leadership team
  • Attend and actively participate in operational meetings as required 
  • Act as super-user and trainer for all medical IT systems
  • Partner with Office Operations Manager to ensure that clinical reporting processes are tracked and followed
  • Maintain quality of services through clinical audits
  • Identification, tracking, resolving, responding to and reporting any quality of care issues
  • Support weekly interdisciplinary meetings
  • Represent Landmark at provider, vendor, facility and health plan meetings as assigned/requested
  • Collaborate with Office Operations Manager for medical supply needs
  • Support and execute corporate initiatives (e.g. centralized quality initiatives)
  • Maintain HIPAA compliance as it relates to member care
  • Telephonic management: Triage clinical phone calls that come into office to ensure appropriate disposition and action
  • People and performance management: Manage direct reports (e.g. NCMs, Ambassadors) as determined by organizational structure in local market.
  • Identify training needs and facilitate education for interdisciplinary team
    • Conduct regular one-on-one meetings and identify development/growth opportunities
    • Provide informal coaching to direct reports
    • Organize and deliver formal training sessions to improve clinical quality and interdisciplinary team coordination
  • Participate in interviewing, hiring, and onboarding of IDT staff   
  • Actively involved with training of new IDT staff
  • Other duties as assigned


  • Current RN License, BSN preferred
  • 5+ years of clinical practice in a hospital, clinic, home care, or nursing home setting
  • 2-3 years of increasing supervisory or management experience in a health care setting
  • Proven ability to lead and coach an interdisciplinary team; strong interpersonal skills
  • Appetite for learning new skills, with continuous improvement mindset
  • Strong computer skills (e.g. Outlook, Excel, Word)
  • Case management or care coordination experience strongly preferred
  • Preference given to candidates with geriatric experience
